For patients with moderate to severe endometriosis-associated pain who do not achieve adequate relief from nonsteroidal anti-inflammatory drugs (NSAIDs) or combined hormonal contraceptives, selecting the next medical intervention requires careful consideration of both mechanism and patient-specific goals. Patients presenting with prominent noncyclic pelvic pain that persists outside of menstruation and does not follow a predictable hormonal pattern may represent a distinct subgroup for whom continued contraceptive trials are unlikely to address the dominant pain component, and in whom earlier escalation to targeted hormonal suppression is worth considering. Oral gonadotropin-releasing hormone (GnRH) antagonists with integrated add-back therapy offer a mechanistically targeted next step, with dosing and timing that can be tailored to the individual patient needs.1,2

Once first-line therapy has been trialed and found insufficient, the focus shifts to structured escalation. A review of which agents were used, at what doses, and for how long provides the clinical grounding needed to select and justify next-step therapy and to set realistic benchmarks for evaluating response.1

Initiation strategies can further support adherence and therapeutic success. Structured treatment planning, including determination of add-back regimen, monitoring intervals, and clinical endpoints, facilitates timely evaluation of response and adjustment of therapy as needed. Long-term extension data demonstrate that improvements in dysmenorrhea, noncyclic pelvic pain, and dyspareunia are sustained with continued treatment. Additionally, that bone mineral density stabilizes after an initial modest decline in the early weeks of therapy and that opioid and analgesic use decreases substantially over time, findings that support structured longer-term management rather than early discontinuation when initial response is partial or gradual.3 Tracking clinical outcomes such as dysmenorrhea severity, noncyclic pelvic pain, and dyspareunia enables data-driven decisions about continuation, dose modification, or additional interventions.2 Coordination across the care team and expanding access to obstetric and gynecologic care can further streamline follow-up and ensure that therapy remains aligned with patient-specific goals, minimizing interruptions, and maximizing long-term outcomes.3

When sequenced thoughtfully, oral GnRH antagonists with add-back therapy represent a mechanistically targeted, adaptable option for patients after first-line therapy failure. Structured initiation and outcome-focused monitoring together support sustained pain reduction, bone health preservation, and long-term quality of life improvements.3
